You are required to complete a conflicts of interest declaration form every
12 months or whenever a personal or business interest comes into conflict.
Declarations will be reviewed to decide how to best manage the conflict of
interest in order to protect you, our customers and our business.
This could include further discussions with your manager and additional
controls being put in place.

If you are unable to access the
electronic version of the Declaration, please complete this off-line version of the
Declaration and email it to ConflictsOfficer@bupa.com.
Your declaration will then be manually added to the Conflicts Register.
